Leave of Absence

If you are, or one of your direct reports is, out of the office for more than three (3) consecutive workdays for medical reasons, or to care for an immediate family member who is ill, notify your HR Representative on the third day. This is not optional.

Employees absent for more than five (5) consecutive days for their own health condition are required to bring a note from their doctor indicating the dates they could not or cannot work and provide a doctor release form before returning to work. 

We are required by federal law to provide forms and information about the Family Medical Leave Act (FMLA) (PDF) and the job protection it provides. We also provide information about disability benefits, paid and unpaid leave, catastrophic leave, accommodations upon return to work, and more.

Please note, a medical condition is private information. We only need a note from your doctor confirming you cannot work and the applicable dates.
